KLPI Photo Consent Form

The purpose of this form is to seek your consent to have your photograph and/or video images taken and used for the purposes detailed below. You have the following options when providing your consent:

• Consent to both photographs and/or video and the inclusion of your name with the image;

• Consent to photographs and/or videos only and not include your name with the image;

• Do not consent to either photographs or video recordings. (Please note: In this nature of work it is very common that professional photographers/videographers will be present at events taking footage)
